Where AI Scribes Break Today

AI scribes are scaling rapidly—but “good enough” documentation can introduce real clinical, legal, and equity risk, especially in specialty and complex care.

Improving the Input: FirstHx

FirstHx addresses this at the source by improving the clinical context fed into a scribe. Patients complete adaptive, evidence-based clinical interviews before the visit, generating structured, longitudinal histories that reduce reliance on incomplete, real-time conversations and allow clinicians to focus on diagnosis and treatment.

Improving Interpretation: CAMH

At the Centre for Addiction and Mental Health (CAMH), the first enterprise AI scribe customized for mental health and addictions deployed in Canada shows where current approaches break—cross-organizational data leakage, missed suicide risk, incomplete mental status capture, loss of non-verbal context, and failure to reflect cultural nuance.

What This Could Enable

Together, these approaches point to a potential shift in integrated care. By combining structured pre-visit histories with AI that can interpret complex conversations, documentation could become more complete and longitudinal—helping clinicians better identify patterns, reduce misattribution, and make more coordinated decisions across mental and physical health.

Cross-Sector Perspective

This session brings CAMH and FirstHx’s unaffiliated work together for the first time. Moderated by the eMental Health International Collaborative (eMHIC), it explores both the benefits and the limitations of cross-sector collaboration—and what becomes possible when different parts of the system are examined together, without assuming partnership.

Presented by:

Dr. Tania Tajirian is Chief Health Information Officer and Chief of Hospital Medicine at the Centre for Addiction and Mental Health (CAMH).

As CHIO, Tania has led a clinically driven, research-informed strategy to reduce documentation burden and optimize workflows through co-design, governance, and responsible implementation of digital tools. She is the principal investigator of CAMH’s multiyear EHR and AI initiatives, advancing practical approaches to workflow integration, trust, evaluation, and responsible AI adoption.

As Chief of Hospital Medicine, she leads research and quality improvement efforts to better integrate physical and mental health care for patients with complex needs, and contributes to national digital health strategy through leadership with Digital Health Canada.

Dr Chris O’Connor, CEO of FirstHx, critical care physician at Trillium Health Partners, and Founder and former President of Think Research.

Chris brings a rare combination of frontline clinical experience and health technology leadership. His work has focused on addressing gaps in clinical workflows through structured, evidence-based approaches, including pioneering digital order sets inspired by aviation safety checklists.

At Think Research, he led from inception in 2006 to its growth into a public listing in 2020, with solutions across 11,000 healthcare facilities worldwide. At FirstHx, he is leading the company’s growth and international expansion, applying his experience in clinical practice and company-building to improve how patient data is captured and used across care settings.

Linda Hanane Mouhamou (she/her/elle), Management Consultant and Patient Partner

Linda has nearly a decade of experience across the mental and digital health sectors. As an independent consultant, she leads strategy, operations, and implementation at the program, organizational, and system level, supporting organizations including the Mental Health Commission of Canada, Canadian Mental Health Association, the Royal Canadian Mounted Police, and CAMH. Her work sits at the intersection of business, equity, lived experience, and digital.

She is a Global Advisor and former Head of Strategy & Partnerships at the eMental Health International Collaborative (eMHIC), a cross-sectoral network of folks working directly or indirectly in digital mental health.

The paper’s central argument is simple: the bigger risk now is moving too slowly. Clinicians are already using generative tools for documentation and information retrieval. Patients are using them to get explanations and navigate a system that often fails to do either. Pretending we can pause this adoption doesn’t make healthcare safer. It may lead to a shadow system.

Will examines where generative AI offers real leverage against familiar constraints, why the complement-versus-substitute distinction matters for clinical practice, what early Canadian implementations are actually showing, and what policymakers and regulators need to get right. The audience should expect a direct, evidence-based case for moving faster with appropriate safeguards not another cautious call for more study.

Will Falk spent 25 years as a strategist and advisor in New York and Toronto advising top academic centres, governments, and innovative companies in healthcare. Since retiring from the PwC partnership in 2017, Will had had several roles. He is an Executive in Residence at the Rotman School of Management at the University of Toronto where he has taught since 2008. He is a Senior Fellow at the CD Howe Institute, a Fellow with the Canadian Standards Association Public Policy Centre, and an Innovation Fellow at Women’s College Hospital Institute for Health System Solutions and Virtual Care. He invests in and has worked with several startups focused on healthcare.

Over six months, adoption, staff experience, and workflow dependencies were analyzed. Findings revealed critical insights into digital readiness and change management. Key lessons highlighted the need for clear expectations, staff engagement, and seamless integration with booking processes which are foundational for sustainable, patient-centred digital transformation.

Jennifer Keough, RN, Clinical Operations Supervisor, Teladoc Health Canada

Jennifer Keough is a Registered Nurse and graduate of Memorial University of Newfoundland, Class of 2013. She brings a strong clinical foundation in rural emergency medicine and critical patient transport by both ground and air. Currently serving as Clinical Operations Supervisor at Teladoc Health Canada, Jennifer plays a key role in advancing virtual care delivery nationwide.

She began her journey with Teladoc in November 2023 as part of the pioneering virtual ER (vER) program, contributing to its development before the launch of the first site. Since then, she has been instrumental in expanding the program to over 20 locations across Canada, supporting clinical teams and driving innovation in digital health services. Her leadership reflects a deep commitment to accessible, high-quality care for patients in diverse communities.

Dr. Tracey Parnell, MD, Chair for Primary Care and Rural Medicine

Dr. Tracey Parnell is a rural Canadian physician and researcher with a passion for medicine that empowers patients and providers. She is currently the Teladoc Health Canada Chair for Primary Care and Rural Medicine, while still maintaining an active and diverse clinical practice. She is an Assistant Clinical Professor at the University of British Columbia and was previously the Research Evaluation and Quality Improvement Sector Lead for the Rural Coordination Centre of BC, which works on behalf of the Joint Standing Committee, an entity that advises the BC government and the Doctors of BC.

She has a heart for providing care for underserved communities, especially those in war torn environments; she has been honoured to care for the most injured and vulnerable on the front lines of Ukraine multiple times since 2022. Believing strongly that patients deserve answers to their question of what is wrong and how can it be fixed, her clinical and research work includes restorative reproductive health, particularly PCOS and endometriosis, focusing on addressing underlying issues contributing to fertility.

For over 35 years, HealthPartners has brought workplaces, health charities and the communities of people with lived experience they serve together under one shared purpose – to improve the lives of those affected by illness.

The HealthPartners Health Advisory Network is an online community initiative by HealthPartners designed to elevate the voice of people with lived experience in shaping Canada’s healthcare future. Grounded in the belief that better outcomes start with listening to patients and caregivers, the Network connects individuals who have experienced illness—or cared for someone who has—with learnings poised to influence donors and decision makers to help build a healthier Canada for us all.

Through this initiative, HealthPartners created a structured and accessible way for Canadians to contribute their perspectives virtually— ensuring that those most affected by healthcare decisions are meaningfully involved in shaping them. By bridging the gap between people with lived experiences and the health ecosystem, the Health Advisory Network offers a scalable model for embedding patient insight into system design and evaluation.

Our presentation will highlight how the Network operationalizes patient engagement at scale and share early learnings from our recent findings - highlighting the experiences of over 400 individuals who are part of this network. It will also explore the impact of integrating patient voices into decision-making — from improving the relevance of solutions to building greater trust and accountability across the healthcare system.
