Chronic obstructive pulmonary disease (COPD) affects more than 2.5 million Canadians, half of whom will not be properly diagnosed until they reach an advanced stage of the disease. Clinicians need easier point-of-care access to evidence-based information to support faster, appropriate diagnoses and improve patient health outcomes.
The Evidence2Practice Ontario (E2P) program offers a free EMR-integrated COPD tool for TELUS PS Suite and Accuro QHR (with an OSCAR Pro version to be released this fall) that supports easier access to the information at the point of care.
In a demonstration of the E2P tool for COPD’s key features, you’ll learn more about:
- COPD diagnosis and how the tool supports the confirmation of diagnosis by spirometry and captures results.
- Disease management support and the embedded accessory instruments to measure the degree of disease burden. You’ll also see how the tool tracks acute exacerbations, graphs timing, reviews, and captures severity to determine the risk of future acute exacerbations.
- Medication plans that provide decision support related to recommended pharmacologic treatments.
A primary care clinician and E2P user* will join us to share their experience using the tool with patients, highlighting their favourite features and optimization tips.
E2P implementation support staff will also be on hand to answer your practical and clinical questions.

Angela Coderre-Ball is the Program Manager for Evidence2Practice Ontario at the Centre for Effective Practice, a leading independent behaviour-change partner in helping clinicians navigate an increasingly complex clinical environment. She is also an adjunct professor in the Department of Family Medicine at Queen’s University, supporting the advancement of primary care research. Angela holds a PhD in Neuroscience and a Bachelor of Engineering Physics from Queen’s University. Angela is deeply committed to health systems research and quality improvement and is excited to share how digital tools can enhance the clinician experience and support patient care.
Jeremy Howcroft is the Program Manager for Evidence2Practice Ontario at the eHealth Centre of Excellence, a trusted leader in digital health for primary and integrated patient care. Jeremy graduated from the University of Guelph, receiving his Bachelor of Engineering in Engineering Systems and Computing, and is a Certified Project Management Professional. He is passionate about enhancing care delivery through innovation and digital technology and is excited to share in developing new ideas and solutions to transform healthcare in Ontario.
Pam Delgaty Pam graduated in one of the first Nurse Practitioner classes from Lakehead University in 2000. Prior to that, she had extensive experience working for the federal government in Sioux Lookout and a variety of remote communities in Northern Ontario. She also served as the Medical Officer working onboard ships with the Canadian Coast Guard. After graduation from the NP program, she worked at Norwest Community Health Centre for 10 years prior to moving over to the Lakehead Nurse Practitioner Clinic where she has practiced since 2011. She is presently the Executive Director and continues to have an active clinical role. She is also on the Board of Directors of the Association of Family Health Teams of Ontario and the Ontario Health Primary Care Lead for Quality Improvement representingNorthern Ontario. Pam has always believed the heart of the health care system is primary care. From her diverse experience, she understands the challenge ensuring that access to this care must be equitable to all Ontario residents. She also hopes to represent Nurse Practitioners and their vital role in an evolving landscape of health care delivery.
