Patient attachment remains one of the most pressing challenges facing primary care systems today. Across jurisdictions, growing numbers of unattached patients, increasing administrative burden, and workforce constraints are putting pressure on providers and health systems to find new approaches to improving access and continuity of care.

Once patients are attached, clinicians must quickly review and understand a patient’s clinical history in order to determine the appropriate care pathway. Referral packages, specialist consult letters, hospital discharge summaries, diagnostic reports, and historical records often arrive as large unstructured document sets across multiple systems. For complex patients, clinicians can spend hours reviewing hundreds of pages of PDFs and scanned records to assemble a clear clinical picture before the first visit.

This webinar will bring together primary care and technology leaders to discuss the realities of patient attachment on the ground and explore how emerging AI-enabled tools are helping care teams synthesize fragmented documentation into structured clinical summaries that support intake assessment, pre-visit preparation, and clinical decision-making.

Through a moderated panel discussion, speakers will share insights on where attachment processes break down, where technology can meaningfully support care teams, and what organizations should consider when implementing AI to improve patient access, navigation, and readiness for care.

      Paige Baltessen, Primary Care Manager, Grand Bend Area Community Health Centre (GBACHC) 

      Paige Baltessen is a healthcare leader and Primary Care Manager at the Grand Bend and Area Community Health Centre in Ontario, where she leads frontline operations, system transformation initiatives, and interprofessional team optimization within a rural primary care setting.

      With a background in nursing and a strong focus on health system improvement, Paige has led multiple initiatives aimed at reducing administrative burden, improving patient access, and strengthening team-based care. Her work includes advancing digital health solutions in primary care, optimizing EMR workflows, and implementing innovative models to support patient attachment and coordinated access across the region.

      Tricia Wilkerson, Director of Integrated Care, Amplify Care

      Tricia Wilkerson is a passionate leader in digital health and integrated care who has spent her career transforming how patients, clinicians, and health systems connect. As Director of Integrated Care at Amplify Care, she has been at the forefront of Ontario’s digital health evolution and previously led the design and rollout of the eServices Program, including eReferral. Her work is helping clinicians streamline clinical workflows, improve access to care, and make the healthcare experience more seamless for patients across the province.

      Alissa Taylor, Chief Product Officer, Akinox

      Alissa Taylor is Chief Product and Technology Officer at Akinox, where she leads the development of CarePath, an AI-enabled platform designed to improve care coordination and reduce fragmentation across healthcare systems. She has spent her career building digital health products focused on complex and chronic conditions, ranging from co-founding AI-driven startups to internationalizing chronic condition portfolios at Teladoc Health. Her work sits at the intersection of clinical psychology, human factors, and applied AI, translating complex clinical workflows into scalable technology. She has a proven track record of finding solutions to complex clinical challenges and evolving them to real-world deployments with strong adoption including developing regulated ML condition management technology later licensed to Johnson & Johnson.
