Webinar Wednesday - Supporting Primary Care with Evidence-Based Digital Point-of-Care Tools

The Evidence2Practice Ontario (E2P) program is focused on digitizing quality standards in both primary care and acute care to support clinicians in delivering high-quality care across the province. E2P is co-led by the Centre for Effective Practice, the eHealth Centre of Excellence and North York General Hospital. E2P is funded by Ontario Health as part of the province’s Digital First for Health Strategy.

This presentation will provide a brief overview of the development and implementation of Ontario Health’s Quality Standards for heart failure, anxiety disorders, major depression, diabetes, and COPD into clinical decision support tools embedded into existing primary care workflows. The presentation will then dive into a demonstration of three E2P tools for primary care: COPD for Telus PS Suite, Anxiety Disorders and Major Depression for Accuro QHR, and Heart Failure for OSCAR Pro. Finally, a clinician end-user will join to provide their clinical perspective and experience using E2P tools and change management supports. 

    Presented by

    Angela Coderre-Ball is the Project Manager for Evidence2Practice Ontario at the Centre for Effective Practice, a trusted source for evidence-based tools, resources, and programs to ensure clinicians have the information needed to deliver high-quality care. She is also an adjunct professor in the Department of Family Medicine at Queen’s University, supporting the advancement of primary care research. Angela holds a PhD in Neuroscience and a Bachelor of Engineering Physics from Queen’s University. She is passionate about health systems research and quality improvement and excited to share how digital tools could enhance clinician experience and support patient care in primary care.

    Dr. Sharon Bal is the lead of the Delta Coronation FHO in Cambridge, ON. She has been involved with MHA quality improvement work, including being part of the QSAC for the Anxiety Disorders Quality Standard (HQO), and a member of multiple regional and provincial MHA committees. More recently, Dr. Bal was a member of the Evidence 2 Practice (E2) Anxiety and Depression tool working group, and a CEP clinical lead for the A+D html tool. Dr. Sharon Bal is an Assistant Clinical Professor with the Department of Family Medicine, Michael G. DeGroote School of Medicine, McMaster University, and the Chair of Electives for the undergraduate medical education program. Dr. Bal is former Regional Clinical Lead (Primary Care) with Ontario Health and a member of the Ontario Primary Care Council. Dr. Bal also currently serves as a board director of the Ontario Medical Association. 

    Jeremy Howcroft is the Program Manager for Evidence2Practice Ontario at the eHealth Centre of Excellence, a trusted leader in digital health for primary and integrated patient care.  Jeremy is a graduate of the University of Guelph where he received his Bachelor of Engineering in Engineering Systems and Computing and is a Certified Project Management Professional. He is passionate about enhancing care delivery through innovation and digital technology and is excited to share in the development of new ideas and solutions to transform healthcare in Ontario.
