This presentation explores how gaps in clinical documentation workflows within hospital-based electronic record systems can contribute to patient safety risks. Drawing on a systematic review of the literature, it examines issues involving medication documentation, handoffs and transitions of care, structured EHR data quality, patient identification, and nursing documentation, while highlighting opportunities for workflow redesign and the role of Health Information Management professionals in supporting safer digital documentation practices.

Ana Paula Martins Inácio, MD, is a Brazilian-trained physician with experience in clinical care, health systems management, research, and quality improvement. She completed postgraduate studies in Health Systems Management in Canada and contributed to a Health Information Management project at London Health Sciences Centre focused on documentation quality, data analysis, and process improvement. Her interests include digital health, patient safety, health information, and the use of data to improve healthcare quality and clinical practice.
